In partnership with Yahweh Care (Registered Provider), Yarning Crescent offers comprehensive Transition Out Services to both Indigenous and non-Indigenous clients. Whether you’re leaving hospital or navigating the paperwork to access the NDIS, our partnership ensures a seamless, supportive, and culturally inclusive experience.

Covering areas from Bondi to the Blue Mountains, we are committed to helping all clients transition safely and efficiently into the next stage of their care journey.

How We Support You

  1. Transition Out of Hospital
    • Yahweh Care is the main point of contact for hospitals and clients, ensuring a smooth and stress-free transition from hospital to home.
    • For Indigenous clients, Yahweh Care collaborates with Yarning Crescent to provide culturally safe and respectful support tailored to specific needs.
    • Non-Indigenous clients are also fully supported with the same level of care and attention, ensuring a personalised approach for everyone.
  2. NDIS Access and Paperwork
    • We simplify the NDIS application process by managing all required paperwork and guiding clients through each step.
    • Yahweh Care ensures efficient coordination of NDIS support to reduce stress and minimise delays.
  3. Simplified Documentation
    • Consent forms are designed to cover both organisations:
      • Yahweh Care (YC1)
      • Yarning Crescent (YC2)
    • This ensures a seamless and transparent process for clients, hospitals, and providers.
  4. Regular Feedback for Improvement
    • We actively seek feedback from hospitals and clients to ensure our services meet and exceed expectations.
    • This feedback helps us continuously refine and enhance our support for all clients.
